Sydney United JDP – Participant Waiver & Consent
By registering for and participating in the Sydney United Junior Development Program (JDP), I acknowledge and agree that:
I understand that volleyball and associated training activities involve physical activity and carry inherent risks, including the possibility of injury. I voluntarily consent to the participant taking part in the program and acknowledge these risks.
I confirm that the participant is physically able to participate and that I will advise Sydney United Volleyball Club of any relevant medical conditions, injuries or other information that may affect their safe participation.
In the event of an injury or medical emergency, I authorise Sydney United Volleyball Club coaches, staff or representatives to provide or arrange reasonable first aid and, where necessary, seek appropriate medical assistance. Any associated medical or ambulance costs remain the responsibility of the participant and/or parent/guardian.
Participants are expected to follow all reasonable instructions from coaches and staff and comply with Sydney United Volleyball Club, venue and applicable Volleyball NSW/Volleyball Australia policies and codes of conduct. Sydney United reserves the right to remove a participant from a session where their behaviour presents a safety risk or significantly disrupts the program.
I understand that Sydney United Volleyball Club will take reasonable steps to provide a safe training environment; however, to the extent permitted by law, I acknowledge the inherent risks associated with participation in sporting activities.
For participants under 18 years of age, I confirm that I am the participant’s parent or legal guardian and have authority to provide this consent on their behalf.
By completing registration, I confirm that I have read, understood and agree to the above Participant Waiver & Consent.
